Fire Safety & Inspection Services LLC is committed to helping you ma'ntain a compliant training environment free of preventable hazards. In compliance with NFPA 1402 and 1403 standards, we propose to conduct a five-year visual evaluation of Building 42, a two-story concrete structure with Class A and gas -fired props, as well as Building 41, a six -story tower with Class A and gas -fired props. Additionally, we plan to conduct an annual evaluation of Building 44, a two-story concrete residence containing only gas -fired props. All three structures were constructed in 2018. Scope of Work: Evaluation requirements for Live Fire Training Structures in accordance with NFPA 1403, Chapter 6, Gas -Fired Live Fire Training Structures and Chapter 7, Non -Gas -Fired Live Fire Training Structures. Our inspection includes a visual evaluation of all structural elements, including building frame, structural f oors, doors, roof scuttles, shutters, stairs, platforms, railings, handrails, and specialty anchors such as rappelling rings and ladder frames. We will also evaluate the burn room linings, lining anchors, and overall integrity of the structure. NFPA requ;res the removal of a representative portion of linings during specified periodic evaluations to allow for a thorough evaluation of the conditions behind the linings. Excessive damage may also require lining removal to evaluate hidden conditions. Unfortunately, we are unable to assist with the replacement and/or repazr of materials removed during the evaluation process. Please note that the evaluation does not include soil testing, concrete testing or core drilling, material testing; or, structural analysis of building design, member or connection sizing, load capacity or adequacy, or building code compliance. Non-structural elements such as burn props, gas burners, and temperature monitoring systems are also not included in the scope of our evaluation. Round Rock Fire Department Round Rock, TX 78664 November 29, 2023 Our Report: Our report will address all elements of the LFTS that fall within the scope of Chapters 6 and 7 of NFPA 1402 and 1403 LFTS evaluation requirements. We will record any deficiencies and damage to structural elements that we observe during our site visit. The report will include photos of the deficiencies and damage observed, as well as general recommendations for corrective actions to be taken to address the issues identified. This will provide the owner/operator with full documentation of current conditions as they exist at the time of our visit, presented in a clear and easy -to -understand format. The document may be used to begin sourcing repairs and upgrading the LIFTS. If the extent of an instance of damage or deficiency is not clear without further analysis, we will inform you of the need for additional inspection. In cases where the damage or deficiencies appear to present an immediate threat to occupants of the LFTS, we will notify the structure's owner/operator before concluding our visit. Operator Responsibilities: To ensure a comprehensive evaluation, we kindly request that the LFTS operator provides us with the building's history and a record of all repairs that have been carried out. We also ask to be informed of any major repairs, alterations, additions, or planned changes before the evaluation begins. In addition, we would appreciate the opportunity to meet with the main point person responsible for the LFTS prior to the evaluation to discuss how the structure is utilized and to obtain a recap of its history. Consulting Fee for Basic Services: Our consulting fee for the evaluation of these structures is as follows: Building 42 five-year evaluation - Six Thousand Five Hundred dollars 6 S00 ; Building 41 five-year evaluation Six Thousand Five Hundred dollars ($6,500); Building 44 annual evaluation Two Thousand Five Hundred Dollars ($2,S00). Reimbursable expenses, such as outside experts, core drilling costs, and equipment rental, will be invoiced at cost plus 15%. A A 4% discount ($620) will be applicable to the invoice total if the on -site portion of work, as outlined in the scope, is completed during travel in progress. Billing Terms: Our billing terms for this project are as follows: Payment is due within 30 days of the date of our site visit. Late payments are subject to 1.5% per month. We reserve the right to terminate the contract if payments are not received in a timely manner. Payment: To streamline the payment process, we offer the convenience of credit card payments for all invoices and a link to payment will be submitted along with our invoice. If credit card payment is not feasible, we kindly request contact information for the accounts payable person(s) responsible for our invoice prior to the start of our evaluation. Terms of Proposal: This proposal is subject to acceptance within (30) days of its date. The evaluation is typically scheduled within (90) days of its date. @2023 Fire Safety & Inspection Services Lt_C Page 2 of 3 Round Rock Fire Department Round Rock, TX 78664 November 29, 2023 Scheduling: Once we receive written acceptance of this proposal, we will work with you to coordinate a mutually agreeable date for our site visit. Please note that for safety reasons and to ensure the highest quality of evaluation, the LFTS must be idle during our evaluation and no training or other activities involving the structure may take place while we are on -site. Any delays or damage to our equipment caused by activities during our evaluation will be the responsibility of the client and invoiced accordingly. Miscellaneous Conditions: It is imperative that the last live fire training evolution have been concluded no less than one day prior to our arrival to ensure the quality of the photos and safety of our evaluator. All debris, Class A fuel materials, and such must be removed so that we can evaluate the walls, ceilings, and floors in their entirety. If the evaluation is delayed or we are asked to suspend the evaluation after we have begun it, we will be entitled to receive payment for any additional time and expenses incurred during this hold period in addition to the fee quoted above. If Fire Safety & Inspection Services LLC is required to cancel the evaluation or return later, we will be entitled to reimbursement for all costs incurred prior to the cancellation, and the rescheduled evaluation will be additional at the quoted price above.
