purposes, this change proposal also served to provide the additional information
requested by CDM for Request for Deviation No. 9 (RFD#9). As documented in
Construction Meeting No. 19, conducted August 31, 2010, CDM has reviewed Pepper-
Lawsori s revised change proposal related to RFD#9 and find it reasonable. Accordingly,
this letter shall serve as CDM's recommendation that Change Proposal No. 10 be included
in an upcoming change order to the Contract for a$38,188.14 credit.

Per our previous discussions, our site utility subcontractor, Joe Bland Construction, would like to use 3000 psi
fiber reinforced flowable fill in lieu of the reinforced concrete shown on Detail BK/SZ-2. Please reference the letter
from Joe Bland Construction, Attachment B.1 and the flowable fill mix design information, Attachment B.2 and the
forthcoming submittal 03300-009-A that will contain additional information. Please also reference the
correspondence with Dennis Dechant with Dechant Infrastructure Services, Attachment A.1 & A.2. Mr. Dechant
provided the design calculations for the steel pipe being provided by Mid-America Pipe and confirms the use of
the proposed flowable fill as adequate for his submitted design calculations. PLC understands this request for

Change Proposal Request No. 13-Clearwell Sample Tap Manhole
File No.: L-EO-0023
Dear Mr.Thuss:
As a follow-up to the October 13,2010 construction progress meeting, attached please find
the drawing modifications depicting a clearwell sample tap as requested by the Owner.
Assuming that this change is still requested by the Owner, CDM recommends that this
change proposal request (Change Proposal Request No. 13) be issued to Pepper-Lawson
(PLC) to include all work described herein. Due to the time sensitivity of the clearwell
pipe penetration work being completed prior to the clearwell manufacturer installing the
prestressed reinforcement system, CDM further recommends that the Owner authorize a
formal authorization for Pepper-Lawson to proceed with this work with a maximum not
to exceed of$4,500. This amount will be negotiated as a lump sum change to the Contract
that is mutually agreeable between Pepper-Lawson and the Owner/Engineer. This work
should be considered as part of the directed work described in the Bid Form (Section
00300),Bid Item 7,and Section 01026 (Measurement and Payment).
If you have any questions, please do not hesitate to contact me at(512)346-1100.
