(HDR) is pleased to submit this proposal for professional engineering services in connection with an evaluation of a fluoride delivery system for the Brushy Creek Regional Utility Authority. The purpose of the project is to evaluate the feasibility of developing a fluoride system to deliver fluoride to the City of Cedar Park delivery point at the intersection of Bagdad Road and New Hope Drive. Our services include the tasks described in the accompanying Exhibits. We propose to provide these services based on a lump sum basis for an amount not to exceed $10,000. If it becomes apparent that the compensation amount will be exceeded, HDR will give written notice of thereof for BCRUA's consideration. Review and comment on all deliverables. OWNER will provide a single set of consolidated OWNER review comments on all deliverables. B. Provide ENGINEER in a timely manner with all criteria and full information as to OWNER'S requirements for the Project, including design objectives and constraints, space,capacity and performance requirements,flexibility, and expandability, and any budgetary limitations; and furnish copies of all design and construction standards which OWNER will require to be included in the Drawings and Specifications; and furnish copies of OWNER'S standard forms, conditions, and related documents for ENGINEER to include in the Bidding Documents,when applicable. C. Furnish to ENGINEER any other available information pertinent to the Project including reports and data relative to previous designs, or investigation at or adjacent to the Site. D. Following ENGINEER'S assessment of initially-available Project information and data and upon ENGINEER'S request,furnish or otherwise make available in a timely manner such additional Project related information and data as is reasonably required to enable ENGINEER to complete its Basic and Additional Services. E. Authorize ENGINEER to provide Additional Services asset forth in this Agreement as required. 1/22/2016 FN EXHIBIT B ENGINEERING SERVICES GENERAL The purpose of this project is to evaluate the feasibility of adding a fluoridation system at or downstream of the City of Cedar Park delivery point near the intersection of Bagdad Road and New Hope Drive. ASSUMPTIONS 1. The fluoridation system will deliver a design dose of to 0.7 mg/L. 2. The maximum flow rate through the delivery point is 15 million gallons per day(MGD). 3. The fluoridation delivery system will utilize hydrofluosilicic acid and will be housed in a stand-alone building. BASIC SCOPE OF SERVICES 1.0 Develop a conceptual layout and design for the fluoridation delivery system and associated stand-alone building. 2.0 Determine candidate locations for the building and delivery system. 3.0 Prepare a conceptual fluoride delivery and injection plan for each candidate location. 4.0 Develop planning-level cost estimates for each delivery system alternative. 5.0 Prepare and submit draft and final versions of a Technical Memorandum documenting the alternatives analysis. ADDITIONAL SERVICES The following Additional Services are not included in the Scope of Services and will not be performed unless specifically authorized by the OWNER: 1.0 Real estate acquisition or appraisal services. 2.0 Preliminary engineering of the selected fluoridation system concept. 3.0 Survey, environmental, biological, historical, and cultural field investigations. 4.0 Services resulting from significant changes in the scope, extent, or character of the portions of the Project designed or specified by ENGINEER or its design requirements including, but not limited to, changes in size, complexity, OWNER'S schedule, character of construction, or method of financing; and revising previously accepted studies, reports, Drawings, Specifications, or Contract Documents when such revisions are required by changes in Laws and Regulations enacted subsequent to the Effective Date of this Agreement or are due to other causes beyond ENGINEER'S control. 5.0 Furnishing services of Consultants for other than Basic Services. 6.0 Preparing to serve or serving as a consultant or witness for OWNER in any litigation, arbitration, or other dispute resolution process related to the Project. 1/22/2016 FN EXHIBIT C WORK SCHEDULE A Draft TM will be completed with 14 days of Notice-to-Proceed. A Final TM will be completed within 14 days of receiving comments from OWNER. 1/22/2016 j�� EXHIBIT D COMPENSATION Total compensation for Basis Services set forth in Exhibit B is estimated to be$10,000. OWNER shall pay ENGINEER for Basic Services set forth in Exhibit B on the basis of Lump Sum as described in Paragraph 1.0. 1.0 OWNER shall pay ENGINEER for Basic Services set forth in Exhibit B as follows: A. A Lump Sum amount of$10,000. B. The portion of the Lump Sum amount billed for ENGINEER'S services will be based upon ENGINEER'S estimate of the percentage of the total services actually completed during the billing period. C. ENGINEER shall not exceed the total Lump Sum amount unless approved in writing by the OWNER. D. The Lump Sum includes compensation for ENGINEER'S services and services of ENGINEER'S Consultants, if any.
