BCRUA_R-21-01-27-7B RESOLUTION NO. R-21-O1-27-7B WHEREAS, the Brushy Creek Regional Utility Authority, Inc. (`BCRUA") has previously entered into a contract ("Contract') with Excel Construction Services, LLC for the BCRUA Phase 1 C Water Treatment Plant Expansion Plant Project; and WHEREAS, the BCRUA has determined that it is necessary to make changes to the quantity of work to be performed or materials, equipment, or supplies to be provided, and WHEREAS, it has been determined that it is necessary to change said Contract in accordance with the attached Contract Change Order No. 4, Now Therefore BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE BRUSHY CREEK REGIONAL UTILITY AUTHORITY, That the Board President is hereby authorized and directed to execute on behalf of the BCRUA, Change Order No. 4 to the Contract with Excel Construction Services, LLC, a copy of same being attached hereto as Exhibit "A" and incorporated herein for all purposes. Mortarcrete does not require a top coat after installation. 2. Since the south, exterior wall of Sedimentation Basin 3C will become an interior basin wall in a future phase,it was determined that the specified exterior concrete coating (Tnemec Series 156 Enviro-Crete) is not required on the south wall. Coat the east,exterior wall of Sedimentation Basin 3C as specified to match the adjacent,existing wall. It is the Construction Manager's and Engineer's opinion that this Field Order does not alter the Contract Price or the Contract Time. Notify the Construction Manager prior to commencing the Work and submit a Change Proposal if the Builder does not agree with the Construction Manager's and Engineer's opinion. Detail Desc. During a site visit on 9/1/20,the Engineer noticed that the inlet baffle wall of the Sedimentation Basin 3C was drawn incorrectly.The baffle wall sections illustrated on Sheet SA-5,Section Al and Sheet MA-11,Section G1 should have been reversed.As a result,four 10"baffle holes need to be plugged on the south side of the wall and four 10" baffle holes need to be added on the north side of the wall as indicated in attached Sheet SA-5. Complete proposed modifications as follows: New baffle holes: Core four new 10" diameter holes in line with existing holes as indicated on attached drawing. If rebar is exposed in new holes,notify RPR and Engineer. Engineer will determine appropriate means of protecting exposed rebar,such as coating. Existing baffle holes: Plug four existing 10"diameter holes adjacent to slide gate TSC-SLG-SB301 as indicated on attached drawing. Install 10"Sch 80 PVC plugs on inlet side of baffle wall. Do not solvent cement plugs into baffle holes. Include threaded SS eye bolt into center of each plug end to assist removal by Owner. Flex LV PURe is not designed for void filling and must be used in compression. If site temperatures are extremely low, heat bands or heated water baths may be used on the pails before and during installation to maintain the product's temperature.Avoid splashing water into open containers,as the material is water activated.Avoid exceeding 90°F when warming. CAUTION:pH NOTICE. Water used to activate PURe Grouts must be in the pH range of 3-10 for optimum foam quality.
