CONTRACT NO.
Field survey embankment placed by the Palm Valley Lutheran Church within the proposed
US 79 right of way.
Adjust 16" waterline crossing US 79 west of existing FM 1460. Adjustment includes new
crossing, casing, fire hydrant relocation and water valve placement.
Additional bridge design cost to the westbound US 79 bridge at Arterial A. The westbound
bridge width was increased from 40' to 58'. Original scope work was for the eastbound and
westbound bridges to be the same width; therefore some of the same design could be used on
both bridges. But since the width of the westbound bridge was increased for traffic control
purposes, the bridge design was not the same.

Supplemental Agreement No. 4 covers the additional services as follows: (A)
Additional survey along Park Valley Lutheran Church/Existing US 79 right of
way for embankment placed within the future right of way for the project
(B) 16" waterline adjustment on the west side of FM 1460 crossing under the
existing US 79 needs a new crossing so that casing will extend beyond the new
pavement limits and relocation of fire hydrants and water valves; (C) Changing of
the westbound bridge width from 40' to 58' increased design cost because both
the eastbound and westbound were originally the same width and therefore some
of the bridge design could be applied to both bridges.
Total cost of Supplemental Agreement No. 4 is $33,266.00 with project
management cost included. Bringing the total cost of the project to $684,862.00.
Funding:
Cost: $33,266.00
Source of funds: RR Transportation System Development Corp
Outside Resources: Carter - Burgess, Inc.
Impact/Benefit: Approval of Supplemental Agreement No. 4 will allow for finite detailing
as far as nearness of US 79 to the Palm Valley Lutheran Church, for
adjustments to the waterline and bridge and will include project
management when project goes to construction.
