WHEREAS, the City is in need of a drug screening collection
site, and
WHEREAS, First Care Medical Clinic ( "First Care ") has agreed to
act as the drug screening collection site, and
WHEREAS, First Care has provided the City with a Letter of
Agreement which the City wishes to approve, Now Therefore,
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF ROUND ROCK,
TEXAS,
That the Mayor is hereby authorized and directed to execute on
behalf of the City a Letter of Agreement with First Care Medical
Clinic to act as the drug screening collection site for the City, a
copy of said Letter of Agreement being attached hereto and
incorporated herein for all purposes.

DRUG /ALCOHOL SCREENING COLLECTION
LETTER OF AGREEMENT
The First Care Medical Clinic located at 900 Round Rock Avenue in
Round Rock, TX hereafter known as "Clinic" does hereby agree to
serve as a drug screening collection site for City of Round Rock
employees.
Under this agreement, the Clinic will collect urine specimens using
chain -of- custody protocol and forward all specimens to the
reference laboratory designated by the City of Round Rock. The
reference laboratory will be responsible for the actual screening
process including the distribution of results to the City of Round
Rock Human Resources Department or other designated entity.
The Clinic will maintain an appropriate supply of drug screening
collection kits according to City of Round Rock guidelines and
insure that all appropriate staff is properly trained in chain -of-
custody drug screening procedures.
The fee for this service is $15.00 per patient and shall be billed
directly to the following address:

The term of this Agreement shall commence on the date set forth
below and shall continue in effect for a period of one year and
shall automatically renew for one -year terms, unless otherwise
terminated.
This agreement may be terminated without cause at any time, by
either party giving the other party 30 ` ays written notice.

ITEM: 9D. Consider a resolution authorizing the Mayor to
enter into an agreement with First Care
Medical Clinic as a drug screening collection
site.
STAFF RESOURCE PERSON: Joanne Land /Linda Gunther
STAFF RECOMMENDATION: With the adoption of the Personnel
Policies and Procedures, one of the changes will include drug testing.
To utilize proper testing and chain of custody procedures and to
assure accuracy along with confidentiality, the staff recommends the
collection site for all testing be conducted by First Care Medical Clinic
located at 900 Round Rock Avenue.
ECONOMIC IMPACT: $15.00 per applicant.
