Martin, P.E., Director of Transportation Services Justification: The scope of work includes the observation and testing to the following construction materials: Laboratory testing, in place field nuclear density testing, and proof rolling observation; Drilled pier observation; Concrete compressive strength specimen sampling, testing and reporting; and, Hot mix asphalt concrete (HMAC) testing. Funding: Cost: $5, 474.21 Source of funds: Transportation System Development Corporation Outside resources: Raba Kistner Brytest Consultants Inc. Background information: Raba Kistner Brytest Consultants, Inc., provided similar construction materials testing services for the project as well as A.W. Grimes Boulevard Phase I. FIRM: RABA-KISTNER-BRYTEST CONSULTANTS, INC. ("CONSULTANT") ADDRESS: 8200 Cameron Road, Suite C-154, Austin, TX 78754 PROJECT: Forest Creek and Doublecreek Drive Phase II This Supplemental Agreement No. 1 to "Agreement for Consulting Services for Construction Materials Inspection and Testing for Doublecreek Drive and Forest Creek Drive Projects with Raba- Kistner-Brytest Consultants, Inc." is made by and between the City of Round Rock, Texas (hereinafter called the "City") and Raba-Kistner-Brytest Consultants, Inc. (hereinafter called the "Consultant"). WHEREAS, City and Consultant executed an "Agreement for Consulting Services for Construction Materials Inspection and Testing for Doublecreek Drive and Forest Creek Drive Projects with Raba-Kistner-Brytest Consultants, Inc." (hereinafter called the "Agreement") on the 20th day of December, 2004, for consulting services for construction materials inspection and testing for Doublecreek Drive and Forest Creek Drive Projects; and WHEREAS, it has become necessary to amend the Agreement to provide for an additional payment of $5,474.21; and NOW THEREFORE, premises considered, City and Consultant agree that said Agreement is amended as follows: I. Article 1.1, Section 1.1.2.5 shall be amended to reflect a one-time additional payment of $5,474.21 by City to Consultant, which sum is for performance of additional services consisting of construction materials testing, and which sum is in full and final payment of all amounts due and owing under the original Agreement for services performed and for allowable reimbursable expenses associated with Phase III "Implementation, Management and Testing."
