615 E Palm Valley Blvd. We received three bids and Sierra Demoliton was selected as the
lowest bidder.

Our proposal is for the demolition and disposal of two two-story buildings located at 615
Palm Valley Drive. All work will be performed in accordance with TDSHS, OSHA and all
applicable Federal, State and Local Regulations, as well as the "Specification" provided.
All work will be covered by $2,000,000.00 in GL and $1,000,000.00 in Auto and WC
policies.

The following proposal is for the work as described for complete demolition.
Scope of Work
• Complete demolition and disposal of 2 structures including foundations
• Backfill and level to match existing grade in the areas of both buildings and front
of lot (North side)
• Remove concrete slab adjacent to rear building
• All debris and thrash will be removed and site will be left clean
• Transportation and legal disposal will be provided for all waste
Demolition Lump Sum Bid $ 59,000.00

Dear Mr. Larry Madsen,
Sisk -Robb, Incorporated ("SRI") would like to thank you for the opportunity to submit
our proposal on the above referenced project.
SRI will provide all necessary supervision, labor, materials, equipment and supplies
required for the proper execution of the demolition for this project. All applicable
specifications, federal, state, and local regulations will be complied with and is the basis
for this proposal.
SCOPE OF WORK — Asbestos Abatement
The scope of work that is included in this proposal is the removal and proper disposal of
the two two-story buildings located at 615 East Palm Valley Blvd. This proposal
includes the buildings and the concrete slabs directly under the buildings. This proposal
does not include any site preparation work or site utility work.
Asbestos Abatement
We propose to complete this scope of work for Fifty Five Thousand Seven Hundred
Dollars & NO/100 555,700.00.
INCLUSIONS
1) All insurance is "A" rated or better companies.
2) Waste hauling.
3) All PPE and proper safety equipment.
EXCLUSIONS
1) State notification fees.
2) Any asbestos or lead testing or analytical fees.
3) Removal of any items not specifically addressed in the scope of work section
of this proposal.
4) Hydro -mulching or seeding.
5) Site construction fence.
CONDITIONS
1) This proposal includes one mobilization; each additional mobilization will be
$2,250.00.
2) If any asbestos containing materials are discovered during the project then
additional costs will be incurred.
3) This project is based on regular weekday hours with ample parking and
dumpster lay -down area.

Complete Demolition and clean up of Two Two -Story Buildings located at 615 E. Palm
Valley Blvd.
Project Requirements and Information:
o Permitting and Notification with Texas Department of State Health Services
o Environmental Controls are in place before construction begins
o Demolition and Removal of two two-story buildings including concrete foundation slabs
Complete haul off of all trash and construction debris
o Removal of concrete sidewalks up to curbs
o Clean up and grade of front area on north side of project near Palm Valley Blvd. where
building and previous foundation had been removed using black dirt or sandy loam
material
o Fill and grade out area where the two two-story buildings and foundation were removed
using either black dirt or sandy loam material
o Contractor is responsible that all utilities are removed and capped off properly before
work begins
o Curbs and asphalt driveway and parking lots are to remain
o Project once started shall be completed in a 30 calendar day period
o Proof of General Liability Insurance shall be provided by successful proposer before
work begins on the project
o A Mandatory walk thru at the site will be held at 10:00 a.m. on Tuesday June 18`h, 2013
o Asbestos Abatement of Project has been completed
o Proposals shall be submitted to the following by 2:00 p.m. Monday June 24th, 2013
